Medical AI models, like chest X-ray classifiers, are often black boxes, making doctors hesitant to trust them. Even when paired with vision-language models (VLMs) that generate text explanations, the underlying visual model remains opaque. This paper introduces KANEx, a framework that uses Kolmogorov-Arnold Networks (KANs)—which have inherently interpretable spline-based components—to ground VLM reasoning. KANEx also creates KAN-Map, a heatmap method derived directly from KAN models, not gradient approximations. Tested on the MIMIC-CXR dataset, KAN-based architectures improved semantic similarity and produced more faithful saliency maps, boosting visual localization and reasoning quality by 10%. The key insight: by grounding explanations in mathematically interpretable units, we can build more trustworthy medical AI. This could accelerate clinical adoption and reduce diagnostic errors.
